Important details for referrers and funders

NHS

We work in partnership with NHS acute and primary care trusts across the South of England. Clients have access to our unique concentration of specialist therapies and nursing care as part of their rehabilitation journey. Working with the Early Supported Discharge, Community and Re-enablement teams as required, we then facilitate their discharge home or to another suitable destination to continue their rehabilitation.

Partnerships​

We work closely with Chroma, Headway, Surrey and The Stroke Association in Surrey to support clients with planning for discharge from our service and to ensure they are connected with organisations that can offer advice and support.
